Citizenship Required Supporting Documents

ONLY ATTACH COPIES, NEVER ATTACH ORIGINAL DOCUMENTS

All applicants must attach the following 3 items with the N-400 form:

  1. A copy of both sides of your Permanent Resident Card (Green Card).
  2. Two identical color passport photographs, with your name and Alien Registration Number (A-Number) written lightly in pencil on the back of each photo.
  3. A check or money order for the application fee and the biometrics. Write your A-Number on the back of the check or money order. *Applicants 75 years of age or older are exempted from fingerprinting and the biometrics services fee.
